CCPA Notice at Collection for Independent Contractors

CareRev (“CareRev”) collects and uses your personal information, including sensitive personal information, for the business purposes listed in the chart below. We are committed to properly handling the personal information collected or processed in connection with any Healthcare Professional’s access to or use of the Marketplace. Capitalized terms not defined in this notice will have the meaning assigned to them in the Marketplace Terms of Use (“Marketplace Terms” or “TOU”): https://www.carerev.com/terms-of-use.

We will not sell personal information, including any sensitive personal information, we collect about independent contractors or share it with third parties for cross-context behavioral advertising.

We may collect the personal information and sensitive personal information categories listed in the tables below. The tables also list, for each category, our collection and use purposes. 

For each category below, CareRev may retain the data as reasonably necessary to comply with applicable law and fulfill the Business Purpose as well as any other purpose for which you provided the data.

California Customer Records occupation-related and personal information, such as your name, signature, Social Security number, physical characteristics or description, photograph, address, telephone number, passport number, driver’s license or state identification card number, insurance policy number, education, current employment, employment history, membership in professional organizations, licenses and certifications, bank account number, credit card number, debit card number, or any other financial, medical or health insurance information. Same purposes as for identifiers category.
Protected classification characteristics under California or federal law, such as age (40 years or older), race, color, ancestry, national origin, citizenship, religion or creed, marital status, medical condition, physical or mental disability, sex (including gender, gender identity, gender expression, pregnancy or childbirth and related medical conditions), sexual orientation, reproductive health decision-making, military and veteran status, or genetic information (including familial genetic information). Conduct internal audits and investigate complaints, grievances, and suspected violations of CareRev’s Community Guidelines (if applicable).
Exercise or defend the legal rights of CareRev and its employees, customers, contractors, and agents.
Commercial information, such as transaction information, purchase history, and financial details. Respond to law enforcement requests and as required by applicable law or court order.
Biometric information, such as facial recognition, fingerprints, iris, or retina scans. Exercise or defend the legal rights of CareRev and its employees, customers, contractors, and agents.
Authenticate users via mobile device API such as Face ID or Touch ID.
Internet or other similar network activity information, including all activity on CareRev's information systems (such as internet browsing history, search history, intranet activity, email communications, social media postings, stored documents and emails, usernames, and passwords) and all activity on communications systems (such as phone calls, call logs, voicemails, text messages, chat logs, app use, mobile browsing and search history, mobile email communications, and other information regarding use of company-issued devices). Facilitate the efficient and secure use of CareRev information systems.
Ensure compliance with CareRev information systems policies and procedures.
Comply with applicable state and federal laws.
Prevent unauthorized access to, use, or disclosure or removal of CareRev's property, records, data, and information.
Conduct internal audits and investigate complaints, grievances, and suspected violations of CareRev’s Community Guidelines (if applicable).
Exercise or defend the legal rights of CareRev and its employees, customers, contractors, and agents.
Geolocation data, such as the time and physical location related to use of an internet website, application, or device, and GPS location data from mobile devices of contractors. Improve safety of independent contractors, customers, and the public regarding use of CareRev property and equipment.
Prevent unauthorized access, use, or loss of CareRev property.
Improve efficiency, logistics, and supply chain management.
Conduct internal audits and investigate complaints, grievances, and suspected violations of CareRev's Community Guidelines (if applicable).
Sensory and surveillance data, such as COVID-19 related temperature checks and call monitoring and video surveillance. Comply with applicable state and federal laws, including on workplace health and safety.
Prevent unauthorized access, use, or loss of CareRev property.
Improve customer service.
Exercise or defend the legal rights of CareRev and its employees, customers, contractors, and agents.
Occupation-related information, such as work history, academic and professional qualifications, educational records, references, background check, drug testing results. Maintain records and comply with record retention requirements.
Communicate with you.
Comply with applicable state and federal labor, employment, tax, workplace safety, and related laws.
Prevent unauthorized access to or use of CareRev's property, including its information systems, electronic devices, network, and data.
Conduct internal audits and investigate complaints, grievances, and suspected violations of Community Guidelines (if applicable).
Exercise or defend the legal rights of CareRev and its employees, customers, contractors, and agents.
Process your registration and participation in the Marketplace as an independent contractor with respect to CareRev.

Compliance with applicable laws, agreements with Healthcare Facilities, and requirements of applicable healthcare accreditation organizations.
Non-public education information, such as education records, degrees and vocational certifications obtained, report cards, and transcripts. Validate credentials and training needed for certain types of engagements.

Inferences drawn from other personal information to create a profile or summary, for example, an individual's preferences, abilities, aptitudes, and characteristics. Engage in human capital analytics, including to analyze data to improve productivity.

Sensitive personal information is a subtype of personal information consisting of specific information categories. We may collect or use information that falls with the sensitive personal information categories listed in the table below to infer characteristics about a person.

Assist with insurance claims process.
Compliance with applicable laws, agreements with Healthcare Facilities, and requirements of applicable healthcare accreditation organizations.
